What to fill in CA NRL form at end of lease

Hi,

I just returned my leased Hyundai Ioniq5.

I was filling up the release of liability form on CA DMV website. They ask the seller and buyer details on this form, as well as the purchase price.

Can someone share what needs to be filled here?

(I got conflicting answers from the Hyundai dealership and Hyundai motor finance)

Here is information provided in response to a similar question a few years ago.

“Here is what Audi FS says to fill into those spots, BMW is likely similar. I would check to see if there’s an FAQ on the BMW FS website with details, that’s where I got the Audi info from.”
