Medical Health Insurance Satisfaction

It’s open enrollment at my wife’s employer so we are looking at switching from Kaiser Gold plan in California to a Platinum plan in either Cigna, Aetna or Anthem/Blue Cross. Cigna is the cheapest, then Aetna then ABC. Why do they charge different rates when offering the same plan? We had the gold plan with Kaiser and it was 70% and $10,000 out of pocket for the two of us. Last year and this year we paid a lot of out pocket since 30% of medical bills is always high, but never got even close the total $10k out of pocket. Gold plan is 80% and $4600 total out of pocket, but almost double the monthly premium, so about $2500 per year more. We are in the 45-60 age group, so more doctor visits.
My questions are, any Recommendations on the above Insurance companies? Customer service? Clain rejections? What plan levels? Silver, Gold, Platinum.

I’m not sure how to even tangentially relate this to leasing a car.
