ETA: Per @mllcb42 's later observation the offers also differ in which models are eligible. Further discussion below.
Last night I received a $1,000 offer, which at first glance I attributed to the most recent request (referenced immediately above) using a secondary email address.
But that wasn’t the case. Looking further:
On 5/13 I received at my primary email address a code worth $1,500.
On 5/16 I signed a lease and received an additional $1,500 incentive by using the code provided.
Email from 5/13 IDs me by first + last name only, which is how I submitted the original request
Email received on 5/28 IDs me by first + full middle + last name, which is how the 5/16 lease was contracted. I have no idea why the dealer did the docs this way (and at the time thought it was odd), but clearly my profile was updated with my full middle name after I used this code on 5/16.
Aha, yes. Eligible vehicle list is different even though the certificate code is the same.
But the terms suggest that the code can only be used once per eligible customer, and I used the code on 5/16.

I was taking at face value the language “per eligible customer” and “per email or postal address.”
It doesn’t say “per transaction” or something similar.
Thinking further, this language doesn’t state that two lessees who have two different postal addresses cannot both apply their own unique codes to the same transaction.
The “immediate family members within the same household” language here only applies to transferring the code from the person named on the certificate to another person.
I signed my lease solo for convenience, but for an extra $1,500 I would have had my spouse get a unique code with another legitimate address, and had both of us put on the agreement.
