It’s asking where to send the title, but it’s not clear whether I need to fill out both the top and bottom sections, or just one or the other. I don’t care if they send it via Fedex, so is the second section “optional”?
The first part is mandatory, put your name and address and sign it.
The second part is optional, only if you want it FedEx’d to you instead of regular US mail.
Funny how there is a ‘checkbox’ there.
